肺动脉高压肺动脉高压pulmonaryhypertension肺循环压力高于正常的病理状态。多并发于心肺疾病,也可由肺血管病本身引起。临床上不少见。肺动脉高压可致右心室肥厚或肺源性心脏病。静息状态下肺动脉平均压>2.67千帕(kPa)(20毫米汞高)或收缩压>4.00kPa(30毫米汞高)。为肺动脉高压。病理生理肺毛细血管和/或左房压力升高、肺血管床总截断面积下降、肺动脉血流量增加、缺氧等是引起肺动脉压力升高的重要因素。当上述因素持续存在并达到一定程度时,肺血管发生形态学改变。肺动脉和静脉中层肥厚,内膜增生,管径变小,增加了肺血管阻力,产生肺动脉高压。病因及分类①原发性肺动脉高压,病因不清。可能与先天性肺小动脉病变、肺小动脉痉挛或自身免疫等因素有关。②继发性肺动脉高压,由已知病因引起,如肺气肿、肺纤维化及肺血栓栓塞;左向右分流的先天性心脏病、二尖瓣病变、心包积液等。临床表现和体征易疲乏,有劳力性呼吸困难、晕厥、咯血等;颈静脉搏动增强,肺动脉瓣区收缩期搏动,肺动脉第二音增强或分裂,有收缩期喷射音和喷射性杂音;右心衰竭后出现相应症状。心电图可出现电轴右倾、肺型P波、右室肥厚和右束支传导阻滞,胸部X射线检查显示肺动脉段突出、右下肺动脉增宽等,超声心动图示右房右室增大及肺动脉高压征象。治疗处理原发病及有关影响因素,选用血管扩张药降低肺动脉压力。
